Add Form Elements

The form elements allow you to collect inputs from users in various ways. Similar to other form building applications, UpPass offer basic elements such as short and long answer, dropdown, choice, file uploads, date inputs for the users. In addition, UpPass also have special inputs that will be relevant for document signing, verification, and suitability test such as electronic signature, address, and dynamic result.

There are 2 types of elements

  1. Common Elements
    • Text Paragraph: Insert paragraph text. This can be used for the contract and terms and condition
    • Short & Long Answer: Fields available to type short and long ended questions and answer fields.
    • Signature: Sign your signature on the signature field with the signature pad option.
    • Drop Down: Add as many options to a drop down list. 
    • Choice: Allowing options to select for chosen questions. 
    • Image: Adding visuals to elements and verification steps.
    • File Upload: The ability to upload files and documentation for evidence on verification steps.
    • Date: Date and time fields available. You can set the year to be in the future or in the past and in B.C or B.E format.
    • Divider: A straight line to separate the section
  2. Preset Elements
    • Full Name: A group of First name / Middle name / Last name fields, Each of these can be customized
    • Gender: Standard preset option- not required 
    • Phone: Relevant phone number field
    • E-mail: Email field option to add a valid email address
    • Address: Address and Country field. 
    • Dynamic Result: You can define the calculation for the final score by selecting the question and assigning a score to each answer choice. You can also assign a score based on a certain keyword in the short answer.
